Custom theme, or sub-theme?

This is the first decision, and it is worth making deliberately rather than discovering later. A sub-theme inherits its parent’s libraries, regions and templates, which means security fixes and improvements keep arriving from upstream. A theme built from scratch owes you nothing and gives you everything — every fix is then yours to make.

Both are defensible. What is not defensible is choosing by accident, because theme incompatibility is one of the most common reasons a Drupal upgrade stalls: a custom theme written against an older core has to be reworked before the site can move.

What the work actually involves

  • Twig templates: template suggestions, preprocess functions, and markup that a content editor cannot accidentally break.
  • Layout systems: Paragraphs or Layout Builder configured so editors assemble pages without needing a developer.
  • Accessibility: focus handling, keyboard navigation, colour contrast and screen-reader behaviour — tested, not assumed.
  • Front-end weight: aggregated CSS and JS, lazy-loaded images, and no library loaded for a feature the page does not use.

Theming is not design, and the difference costs money

A design is a picture of a page. A theme is the system that produces every page — including the ones nobody mocked up: the 404, the search result with a single hit, the article whose author has no photo, the view with forty rows on a phone.

Most theming problems are really content-model problems arriving late. When a template needs six conditionals to cope with the shapes its content can take, the fix usually belongs in how the content is structured rather than in the Twig.
